CHN launches with Prevvy a new remote care program for hypertension patients

The Importance of Patient Engagement and Virtual Care for Hypertension Patients

In January of this year, HHS awarded $90 Million to support health centers in addressing hypertension among racial and ethnic minorities. Community Healthcare Network was one of the awardees that for the next three years will work to improve blood pressure control through the use of remote patient monitoring technology.


A key component of the program is improving the day-to-day care of patients at home. Using Prevvy as their patient platform for remote care, CHN is reaching out to patients where they are, at home, in an effort to have a more frequent interaction with the patient and to remove barriers of access to care.


CHN piloted the program with just a few patients, but starting in October CHN will gradually offer it to all eligible patients with hypertension.


A team of CHN and Prevvy leaders worked together to create a digital care plan that supports patients at home as they start to better manage their blood pressure. Using an evidence-based approach and a Patient-Centered Clinical Decision Support system, the team automated clinical decision rules so both patients and clinical staff receive personalized notifications based on the patient's evolution. Both organizations are committed to track and make public their results as they work together to improve the health of many New Yorkers.
